Remote Community Care Manager & Clinician
Upper Peninsula Health Plan (UPHP)
Upper Peninsula Health Plan (UPHP) seeks a Remote Community Based Clinical Coordinator - Care Manager to assess, plan, and coordinate care for members in the Luce County area. This full-time remote role includes home and facility visits as needed, coordinating services with providers and community-based resources.
The role requires a Michigan-licensed RN with 2+ years of clinical experience; BSN or social work licenses are preferred. Benefits include health, 401(k), loans, tuition, and holidays.
