GENERAL DESCRIPTION - THIS IS AN OPERATIONS WORKFORCE MANAGEMENT ROLE:

Central City Health is committed to enabling every patient in our community to thrive through the delivery of high-quality, integrated, and patient-centered care. The Director, Case Management and Social Health provides leadership and operational oversight for an integrated case management workforce spanning behavioral health, housing, community health, outreach, and social support services. The Director leads care coordination and service navigation across programs to promote continuity of care, reduce fragmented service delivery, and ensure individuals with complex needs receive coordinated, person-centered support. This role oversees case management operations, workforce performance, caseload management, and program fidelity while collaborating with clinical, housing, and organizational leadership to address social determinants of health and improve patient outcomes.

RESPONSIBILITES:

• Own the unified case management model: ensure members with cross-cutting needs (e.g., a member who is both SMI and housing-insecure) get one assigned case manager, not multiple uncoordinated handoffs.
• Directly manage the Case Management Pool, the Outreach & Community Health team and the two Housing Specialists cross-training into full case management roles.
• Partner with Clinical Supervision & Practice and/or the BH Supervisor (LMSW) on clinical/EBP fidelity and licensure-supervision matters for case management staff, pending resolution of the open question over which seat actually holds this authority day-to-day — this role holds administrative authority only. Do not attempt to resolve clinical/licensure questions unilaterally.
• Coordinate directly with the Senior Team Lead and the Administrative Manager of Housing, both under the Deputy CFO, to ensure HUD and housing-program fidelity is maintained as Housing Specialists' caseloads integrate into the broader case management pool.
• Own the Supported Employment (IPS) program via its dedicated team, ensuring fidelity to the IPS model.
• Coordinate with the Chief of Organizational Growth & Development on outreach/community engagement initiatives — a dotted-line relationship, not a reporting change.
• Establish and monitor acuity-weighted caseload standards across the unified case management pool, including redistribution as the two Housing Specialists cross-training into full case management roles' caseloads convert from housing-specific to full case management.
• All other duties as assigned by the Leader.

EDUCATION AND EXPERIENCE:

• BLS certified required
• Master's degree in social work, public health, or related field; Michigan LMSW or equivalent licensure preferred.
• Minimum 5 years leadership experience spanning case management, care coordination, or social services, ideally across both behavioral health and housing/SDOH contexts.
• Demonstrated experience managing organizational change is being created specifically to consolidate a previously fragmented function.
• Familiarity with IPS Supported Employment fidelity, HUD housing program requirements, and 42 CFR Part 2 confidentiality boundaries for any SUD-adjacent case management work.

SKILLS:

• Understanding of the dynamics of individuals who have a history of homelessness, substance abuse issues, and mental illness.
• Ability to clarify assignments, establish priorities and communicate effectively; ability to plan, direct, coach, motivate and evaluate the work of others.
• Demonstrated willingness to assume accountability, make decisions, supervise, coach, teach, counsel, and motivate others.
• Mastery of communication and team building skills necessary to lead meetings and make formal presentations; mastery of writing skills required for grants, proposals, and reports; as well as management of complex dynamics.
• Ability to analyze the impact of Managed Care environment on the Programs and apply concepts to program operations.
• Ability to speak and write effectively using proper grammar and punctuation.
• Aptitude to manage multiple responsibilities efficiently within a broad scope of functional areas.
• Ability to influence others in an advisory/consultative/supervisory capacity.

PHYSICAL DEMANDS/WORKING CONDITIONS :

• Walking, sitting, and/or standing for prolong periods of time may be required
• Normal office and medical office environments with exposure to noise, dust, or extreme temperatures.
• Must be available to work all hours scheduled. These include but are not limited to early mornings, evenings and weekend hours as determined by the organization and Senior Leadership.
• Work hours may shift depending on project or program needs.
• May have close contact with clients and/or their living environments which could include potential exposure to contagious diseases, unpleasant odors and/or to bug/pest infestations.
• Must possess and maintain an acceptable background check clearance, certifications, licensures and/or training as required by Central City Health, federal/state/county agencies, and other entities.
