Registered Nurse (RN) – Home Health

Location: Washington County, MD

We are seeking an experienced and compassionate Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality skilled nursing care to patients in their homes while coordinating care, educating patients and families, and helping individuals achieve the highest level of independence.

This is an excellent opportunity for an RN with Home Health experience who is proficient with HomeCare HomeBase (HCHB) and OASIS documentation.

Schedule

• Monday–Friday
• 8:00 AM – 5:00 PM
• Full-time (40 hours guaranteed)
• Rotating on-call schedule every 3rd weekend

Compensation & Benefits

• Guaranteed 40 hours per week
• Mileage reimbursement: $0.43 per mile

Coverage Area

• Washington County, Maryland

Productivity Expectations

• 30 productivity points per week

Required Qualifications

• Active Maryland RN license or Compact RN license
• Previous Home Health nursing experience
• HomeCare HomeBase (HCHB) experience required
• OASIS documentation experience required

Preferred Qualifications

• Experience with PICC line management
• Wound VAC experience

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Deliver skilled nursing services in the home setting while promoting health maintenance and disease prevention.
• Complete accurate OASIS assessments and maintain timely documentation in HomeCare HomeBase (HCHB).
• Coordinate patient care with physicians, therapists, case managers, and interdisciplinary healthcare teams.
• Monitor patient conditions and communicate changes to the physician to ensure appropriate interventions.
• Administer and monitor medications, reconcile physician orders, and ensure compliance with medication management policies.
• Educate patients and caregivers on medications, disease management, treatment plans, and home safety.
• Supervise and support Licensed Practical Nurses (LPNs) and other clinical staff as appropriate.
• Coordinate hospital admissions, discharges, and follow-up care to ensure smooth transitions.
• Maintain compliance with agency policies, infection prevention standards, and regulatory requirements.
• Participate in the rotating on-call schedule to provide after-hours patient support.
