- Screen patients for social needs, including food insecurity, housing concerns, transportation barriers, utility needs, personal safety, and nicotine use (including youth ages 13-17).
- Identify patients who may benefit from chronic disease prevention or management programs, such as weight management, diabetes prevention, or tobacco cessation.
- Use approved screening tools and patient-centered communication techniques.
- Document all screenings and patient interactions in the electronic health record (EHR) and other required systems.
- Connect patients to hospital services and community organizations.
- Help patients apply for and enroll in public benefit programs such as SNAP, Medicaid, and My Life My Quit.
- Follow up with patients to make sure they were able to access services and address any barriers.
- Support discharge plans by reinforcing medication instructions, follow-up appointments, and transportation needs.
- Work with care managers, social workers, nurses, and providers to support patient care plans.
- Track referral outcomes and complete required internal reports, including monthly reports to the Oklahoma Hospital Association.
- Build and maintain relationships with local community organizations and service agencies.
- Participate in community outreach activities to increase awareness of hospital services and available resources.
