Faith Home Health & Hospice is hiring a licensed Physical Therapist or a Physical Therapy Assistant to support patients in the Wichita, KS area and surrounding communities. This position is a PRN role.

As a Physical Therapist, you will provide skilled in-home therapy services based on individualized treatment plans, working collaboratively with physicians and interdisciplinary team members to maximize patient independence, safety, and quality of life.

Implement and modify individualized treatment plans in collaboration with interdisciplinary team members.

Maintain timely and accurate clinical documentation, including evaluation reports, daily notes, progress updates, and discharge summaries.

Provide education to patients and families regarding safe mobility techniques, adaptive equipment, fall prevention, and home exercise programs.

Communicate effectively with patients, caregivers, physicians, and agency staff to support coordinated, high-quality care.

Participate in case conferences, chart audits, and quality improvement initiatives to ensure regulatory compliance and positive patient outcomes.

Current Kansas Physical Therapist License required.

Graduate of an accredited Physical Therapy program (APTA or AMA approved).

Home Health experience preferred.

Current CPR certification required.

Valid driver's license, current auto insurance, and reliable transportation.

Demonstrated ability to work independently while maintaining strong communication with the care team.
