What You'll Do (Your Role & Impact) Deliver Compassionate Care : Carry out personalized plans of care directed by physicians to keep patients safe and comfortable at home. Prevent Re-Hospitalizations : Actively monitor condition changes to adjust care plans early and minimize hospital visits. Collaborate on Care : Communicate directly with physicians regarding medication needs and ongoing care adjustments. Empower Families : Teach and counsel patients and family members on managing injuries, diseases, and recovery processes. Digital Documentation: Complete patient care records efficiently in our WellSky EMR system. Continuous Growth: Attend orientation, ongoing staff meetings, and earn CEUs. Requirements What We’re Looking For Active Ohio LPN License (in good standing) Current CPR Certification Reliable Transportation & current auto insurance Tech Comfort: Solid computer skills and ability to navigate an EMR system (WellSky experience is a plus!)
