Hours/Shifts:

· Our openings include full-time, part-time 12 hour shifts only 

Day in the Life / Expectations

· Provide patient care to residents

· Comply with state and federal regulations.

· Work collaboratively with members of the healthcare team.

· Serve as a primary coordinator of all disciplines for well-coordinated patient care.

· Assess and coordinate patient's discharge planning needs with members of the healthcare team.

 

Requirements:

· Valid state license(s)

· Excellent verbal communication skills and strong interpersonal skills

· High integrity and well-grounded work ethic

· Agility and flexibility with changing priorities

· Team-oriented and a practical team-player

· Professional demeanor

· At least one year experience providing care and services in long-term care (preferred)
