Pharmacotherapy Pharmacist - Rheumatology/GI - Pharmacotherapy Clinic - Full Time - 8 Hour - Days

Overview The Advanced Practice Pharmacist (APP) in the Pharmacotherapy Clinic at John Muir Health plays a vital role in the comprehensive management of patients with a variety of complex and chronic disease states. Responsibilities Assess medication regimens for safety, efficacy, and adherence Streamline therapies when appropriate Mitigate drug interactions Educate patients and caregivers to support long‑term treatment success Perform clinical assessments and order/interpret laboratory tests Monitor patient responses to therapy to ensure safe, timely, and cost‑effective care Collaborate closely with physicians and the care team to ensure continuity of care and achievement of clinical goals Maintain strong documentation and communication skills in patient‑facing and interdisciplinary settings Contribute to policy development, quality improvement, and staff education Supervise technicians, support regulatory compliance initiatives, and assume leadership duties in the absence of pharmacy management Qualifications Education: Doctor of Pharmacy (PharmD) or Bachelor of Science (BSPharm) from an ACPE‑accredited school Experience Meet California State Board of Pharmacy requirements for Advanced Practice Pharmacist recognition by fulfilling any two of the following: Current board certification in a relevant clinical area (e.g., BCACP, BCPS, BCGP, BCOP, or other board‑approved specialty) Completion of an ASHP‑accredited PGY‑1 or PGY‑2 residency in Ambulatory Care, Pharmacotherapy, Cardiology, Internal Medicine, Oncology, Geriatrics, Specialty Pharmacy, or other applicable specialty At least 1,500 hours of direct patient care experience within the past 10 years under a CPA or similar structured clinical protocol For candidates without residency training, a minimum of five (5) years of clinical experience in ambulatory care or chronic disease state management is strongly preferred Ability to multitask and adapt to a high‑acuity outpatient environment Demonstrated ability to perform in a complex and collaborative clinical setting Experience educating, monitoring, ordering, and following labs for patients related to metabolic disease states Experience developing policies and engaging in continuous process improvement Certifications & Licenses Registered Pharmacist (RPH) – California Board of Pharmacy (required) BLS Basic Life Support – American Heart Association (required) Advanced Practice Pharmacist License (APh) – required within six months of hire Board certification in a pharmacy specialty recognized by the Board of Pharmacy Specialties (required for positions where relevant) Work Schedule Shift: 08:00 – 17:00, 40 hours per week.
