ACT Case Manager: Recovery-Focused Care Lead

Job Overview The ACT Team Case Manager functions as an integral part of the Assertive Community Treatment Team to provide intensive, recovery-based treatment for the highestriskconsumers of mental health resources. The ACT Team Case Manager serves as a primary case coordinatorin order toensureeach client enrolled in ACT is receiving theappropriate servicesrelevant to their recovery goals. Responsibilities and Duties Participate in all aspects of client’s treatment from admission through discharge; serve as a primary source of service coordination for all clients enrolled in ACT; Work collaboratively as part of the ACT Teamin order toprovide comprehensive care that meets the needs of client team members; Respond to and assess emergency situations and incidents; assure thatappropriate actionsare taken to protect the health and welfare of clients and their families; Understand and be capable of performing the functional duties of other staff ACT Team members and be willing to serve as a substitute for the provision of other specialist services when the designated staff team member is unable to do so; Attend daily team meetings, individual supervision, and group supervision as scheduled; Education and Experience Candidates must have the following qualifications: Bachelor’s Degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing,counselingor educationrequired Experience working in a behavioral health setting strongly preferred Full Time Benefits Paid Time Off – PTO (over6 weeksper year) Two (2) Floating Wellness Holidays 401k Retirement Plan (with 200% employer match contribution) Affordable Health Insurance Vision Insurance (free employer-paid coverage) Dental Insurance (free employer-paid coverage) Life Insurance (free employer-paid coverage) Cell Phone Reimbursement (up to $300 annually) *for qualifying positions* #J-18808-Ljbffr
