POSITION SUMMARY

The Clinical Pharmacist in Case Management will work with nurse practitioners and social workers to ensure total patient care under the supervision of a GHC doctor. The Clinical Pharmacist will work to improve patient medication compliance and patients overall quality of life by providing medication counseling. They will promote GHC tools to improve patient care and patient quality of life. The Clinical Pharmacist will deliver comprehensive medication management in an ambulatory setting, addressing adherence, affordability, and therapeutic goals in partnership with patients and providers.

PRIMARY ACCOUNTABILITIES

Reviewing case management patient charts to determine the appropriateness of medication therapy. Coordinate with the pharmacy to improve medication compliance by assessing potential compliance barriers and providing tools to improve compliance. Work with the Case Management team to provide exceptional patient care and education by counseling patients on their medication therapy to include potential adverse events, proper administration techniques, and medication monitoring parameters. Review labs for uncontrolled disease states. Complete MTMs for your designated office location via telephone or in person. Assist with clinical pharmacy team special projects. Provide clinical information to providers on an as needed basis.

PRIMARY TASKS & DUTIES

Coordinate with the pharmacy to reconcile patient medication lists and synchronize medication refills for the patient’s Case Management appointments. Monitor patient compliance and communicate with providers in the EMR to note non-compliance. Assist in onboarding new Coordination of Care patients. Assist with completing MTMs via MTM platforms either by telephone or when the patient is seen for Case Management. Educate GHC providers on new medications as they become available. Act as a drug information resource for GHC providers by providing evidence-based recommendations to provider questions in a timely manner. Collaborate with healthcare professionals to ensure optimal patient care and outcomes. Educate patients on GHC procedures for patient assistance and new GHC technologies as they become available. Educate patients on proper use of medications and diabetes technology to ensure treatment benefit and compliance. Educate patients on appropriate diet/lifestyle modifications for preventing or treating chronic disease states. Review medications and labs to assess uncontrolled disease states. Efficiently use the EMR to effectively note any interventions and care plans for proper documentation in the patient chart. Follow up with patients and/or caregivers with additional follow-up phone calls if needed.

ESSENTIAL FUNCTIONS/KEY COMPETENCIES

Function in a multi-tasking, multiple priorities environment, while maintaining accuracy and attention to detail. Utilize a wide variety of office technology and tools to collect, enter, process, file patient records and documentation. Ability to select, train, supervise, evaluate and discipline employees. Requirements Education Minimum of a baccalaureate degree or PharmD from an accredited school of pharmacy Current, unrestricted license to practice pharmacy in the state of South Carolina (one (1) year of pharmacy resident experience or three (3) years clinical pharmacy experience). EMR experience preferred Possess the ability to perform effectively and handle multiple tasks simultaneously. Ability to keep detailed records. Experience with problem-solving, strong advocacy, and organizational skills. Follows HIPAA, FWA, corporate compliance, and all applicable state and federal laws. Experience working in a nonprofit organization preferred but not required Communication, customer service or working with the general public, preferably in a medical care facility. General computer competence including basic Word, Excel, and potential to be trained on specific software for patient information, billing and communication. Ability to interact with computer screen for up to six hours at a time (visual acuity required). Must have manual dexterity for use of keyboard. Ability to remain stationary for periods of up to four hours.
