Job Title: Case ManagerJob Description

The Case Manager is responsible for providing comprehensive case management services to consumers within the Day Treatment Program. This role entails collaborating closely with the Director and Supervisors to uphold the agency’s mission of offering individual counseling, group therapy, and crisis intervention in a drug-and violence-free environment. The Case Manager is expected to adhere to the policies and procedures of the Office of Mental Health and the agency, reporting any significant concerns to the Clinical Director.

Responsibilities

  • Provide case management services to consumers of the Day Treatment Program.

  • Collaborate with the Director and Supervisors to ensure adherence to the agency’s mission.

  • Uphold a drug-and violence-free day treatment environment.

  • Adhere to the Office of Mental Health and agency policies and procedures.

  • Report significant program or consumer concerns to the Clinical Director.

Essential Skills

  • Case management

  • Ability to travel independently

  • Capability to sit for approximately six hours a day

  • Enforcement of agency policies and procedures

  • Regular attendance

Additional Skills & Qualifications

  • Bachelor’s Degree in Social Work, Mental Health Counseling, Psychology or related field

  • Minimum of one year experience working with MICA population

  • Knowledge of addiction counseling

  • Understanding of regulations governing MICA/Day Treatment Programs

  • Computer literacy

  • Excellent interpersonal, written and verbal communication skills

  • Strong leadership qualities

  • Team player

  • Ability to handle routine and crisis situations with good judgment

Work Environment

The position operates within a Day Treatment Program with working hours from Monday through Friday, 9am-5pm. The role may require a flexible schedule, including some evenings, weekends, and holidays. The environment is drug-and violence-free, promoting a safe and supportive space for consumers and staff.

Job Type & Location

This is a Contract to Hire position based out of Bronx, NY.

Pay and Benefits

The pay range for this position is $24.00 - $29.00/hr.
