Board Certified in OB/GYN [REQUIRED] * no exceptions
CO license (or willing to obtain) [REQUIRED]
Clean malpractice - [HIGHLY PREFERRED]
Most recent NPDB Self Query Report (within 30 days active) [REQUIRED]* no exemption, required at presentation

Annual salary range: $291,000 $324,000 (full-time)
