Managing a caseload of patients, the 100% remote New Jersey Licensed RN Case Manager will focus on kidney health management, delivering education on renal replacement therapy, coordinating care, and promoting high-quality patient outcomes. Key responsibilities Handle inbound and outbound calls to provide exceptional service to members Educate patients on kidney health and related conditions, optimizing their understanding of dialysis and transplant options Collaborate with an interdisciplinary care team to coordinate patient care and address individual needs Required qualifications Active, unrestricted RN license in New Jersey Bachelor's degree required; 6+ years of RN experience with 3+ years in case management may substitute for degree CCM certification preferred Three years of experience in case management preferred Experience in a dialysis or transplant center preferred #J-18808-Ljbffr
