Licensed Master Social Worker

Job Description

We are seeking a compassionate and skilled Licensed Medical Social Worker (MSW) for a temporary assignment supporting patient care needs in a clinical setting. This role focuses on psychosocial assessments, care coordination, and supporting patients and families through complex medical situations, including potential transplant-related care.

The ideal candidate will have strong communication skills, experience in medical social work, and the ability to work effectively in a multidisciplinary environment.

Key Responsibilities

• Conduct comprehensive psychosocial assessments to evaluate patient needs and support care planning
• Provide emotional support, counseling, and resource guidance to patients and families
• Collaborate with physicians, nurses, and interdisciplinary teams to coordinate patient care
• Assist with discharge planning and connect patients to community resources and services
• Support patients through complex medical treatments, including transplant-related processes when applicable
• Maintain accurate and timely documentation of assessments, interventions, and outcomes
• Advocate for patient needs while ensuring compliance with healthcare policies and standards
• Communicate effectively with patients, families, and healthcare teams

Qualifications

• Master’s degree in social work (MSW)
• Licensed Master Social Worker (LMSW) in New York (or ability to obtain within 1 year of hire)
• Experience in medical social work in a hospital or outpatient setting
• Strong knowledge of psychosocial assessment
• Excellent communication and interpersonal skills
• Experience in transplant social work

Important Details:

• 27-week contract position (extensions available)
• Shift is 3x8-hour Variable Day shifts (9:00 AM – 5:00 PM), 24 hrs/week
• Shift Schedule: Monday 9:30 am - 4 pm, Tuesday 9 am - 5 pm and Wednesday 10 am - 3 pm.
• Location is New York, NY 10065
• Pay Range for Locals: $55 - $58 per hour
