Innovative In-Home Brain Health & Wellness Program | Hybrid Position

We are building a pioneering, multidisciplinary Brain Health & Wellness Program designed to bring personalized brain health services directly into clients' homes.

Our mission is to support cognitive health, address modifiable risk factors, and help individuals experiencing cognitive changes optimize brain function, maintain independence, and improve quality of life through a comprehensive, whole-person approach.

Position Overview

We are seeking a compassionate, knowledgeable professional to join our growing team. This hybrid role combines work from home with in-person visits to clients' homes, providing individualized assessments, wellness education, and support in collaboration with a multidisciplinary team.

Preferred Qualifications

• Background in cognitive decline, brain health, dementia care, or healthy aging.

• Experience in nutritional assessments and nutrition-related interventions.

• Knowledge of lifestyle factors affecting cognitive health, including nutrition, exercise, sleep, and stress management.

• Strong communication, documentation, and client-care skills. xhyhwjd
