About the Role:

• Responsibilities:

  • Provide high-quality, patient-centered care to individuals with a variety of medical conditions and needs

  • Administer medications, perform vital signs, and monitor patient conditions as required

  • Develop and implement individualized care plans in collaboration with the healthcare team

  • Assist with daily living activities, such as bathing, dressing, and ambulating patients

  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions

Requirements:

• Education:

  • High school diploma or equivalent required

• Experience:

  • 0 years of experience as an LPN (new graduates welcome to apply)

• Certification:

  • Current LPN licensure in the State of Louisiana

• Other:

  • Ability to maintain confidentiality and adhere to HIPAA guidelines

  • Strong communication and interpersonal skills

  • Physically able to lift and transfer patients as needed
