Job ID:26-21363 Pay Range: Regular Pay: $42/hr Weekly Housing & Meal Stipend: $1680/weekly Weekly Per Diem: $1021.72 Weekly Gross Pay: $ 2,701.72 (subject to completion of scheduled hours)

Key Responsibilities:

• Coordinate and manage patient care from admission through discharge or transfer.
• Assess, plan, implement, and evaluate patient care using the nursing process.
• Collaborate with physicians, nurses, therapists, patients, families, and insurance providers to develop and coordinate individualized care plans.
• Review admissions, discharge planning, observation stays, readmissions, and utilization management to ensure appropriate level of care.
• Initiate and monitor clinical pathways based on admission diagnoses.
• Advocate for patients and families while facilitating communication among the healthcare team.
• Evaluate quality and utilization cases to improve patient outcomes and resource management.
• Maintain accurate documentation using EPIC EMR.
• Support patient transitions to appropriate levels of care and community resources.
• May provide case management support in the Emergency Department as assigned.
• Contract Length: 13 Weeks
• Shift: 8-hour shifts, 8am - 4:30pm, weekends and weekdays
• Scheduled Hours: 40 hours [Hours per Facility]

Key Requirements and Technology Experience:

• Active California Registered Nurse (RN) License.
• Minimum of 1 year of recent Case Management experience in an acute care setting.
• Current BLS/CPR Certification.
• Strong knowledge of discharge planning, utilization review, and care coordination.
• Experience with EPIC EMR preferred.
• EMR: EPIC
