General Responsibilities

1. Organize, coordinate, and supervise day-to-day operations of the Care Coordination Unit.
2. Provide direct supervision and oversight to Care Coordination Staff including hiring, onboarding, training, performance management, and evaluation.
3. Provides clinical support and case consultation to Care Coordinators, advising on challenging cases.
4. Ensure education, training, and recertification of all assigned staff.
5. Support clients in the Community Care Program in the absence of regular Care Coordinator staff and ensure programmatic coverage, otherwise.
6. Maintain cooperative relationships with service providers and community organizations.
7. Create and maintain confidential case records, ensuring compliance with program standards.
8. Complete all required reporting and documentation accurately and timely.
9. Ensures effective outcomes and appropriate outputs for program.
10. Act as an agency ambassador in communities served, maintaining strong knowledge of available programs.
11. Actively support and hold each supervised employee responsible for achieving goals related to job performance, professional growth and development goals, fostering a culture of continuous learning and accountability.
12. Other duties as assigned within the guidelines of this position.

1. Must be an RN, or a BSN, or have a BA/BS degree in social science, social work or related field. One year of program experience, which is defined as assessment, provision, and/or authorization of formal services for the elderly, may replace one year of college education up to and including four years of experience replacing a baccalaureate degree. Master’s Degree preferred but not required.
2. Prior supervisory experience preferred.
3. Experience working with older adults and/or caregivers required.
4. Ability to relate well to persons with disabilities and older individuals.
5. Ability to work effectively in a dynamic, fast-paced, and diverse environment.
6. Proficiency in Microsoft Office applications (Outlook, Word, Teams).
7. Strong verbal and written communication skills.
8. Passage of complete background clearance.
9. Valid driver’s license, reliable transportation, and proof of liability insurance.
10. Adhere to the Agency’s Code of Ethics and conduct all responsibilities in accordance with the standards outlined in the Employee Handbook.
