Position Summary Residential Home Health and Hospice (Residential) is seeking a full‑time Home Health Registered Nurse (RN) to serve as a case manager and collaborate with our multidisciplinary team to deliver patient care in Jackson County, MO, Johnson County, KS, and surrounding areas. The RN will manage and deliver patient care plans and support patients across multiple states. Key Responsibilities Lead and collaborate with care teams to develop and implement patient care plans. Provide home health nursing services to patients in designated regions. Ensure compliance with charting, documentation, and critical deadlines. Self‑manage time and schedule, driving 20% of the day to patient locations. Utilize tablets, mobile phones, and EMR software for clinical documentation. Participate in 4-month orientation pay guarantee and clinical outcome bonuses after one year. Attend on‑site or virtual case conferences as required. Qualifications RN license in Kansas and Missouri or a multistate license covering those states. Graduate from an approved nursing school. At least one year of RN experience with clinical competence. Valid driver’s license and ability to travel to patient sites. Commitment to meet critical charting deadlines. Self‑management and time‑management skills. Experience with tablets, mobile phones, and electronic medical record (EMR) software. Successful completion of a pre‑employment drug screen (including medical and recreational marijuana testing). Compensation & Benefits Competitive per‑visit compensation model with 4‑month orientation pay guarantee. $20,000 total bonus potential: $10,000 first‑year success bonus and $10,000 sign‑on bonus (paid within first 18 months).
