Registered Nurse - Case Manager

Position Overview We are seeking a dedicated professional to join our team for a full‑time role with a schedule of 5/8s, including every other weekend. This position requires a California RN License and BLS certification, along with a minimum of 5 years of RN Case Management experience. Key Responsibilities Care Coordination and Transitions Planning: Collaborate with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely patient progression and transition to appropriate care levels, aiming to prevent unnecessary admissions or readmissions. Patient Assessment: Review initial physician admission care plans, gather additional information, and conduct screenings for ancillary supportive services. Utilization Management: Ensure patient records meet level‑of‑care requirements, expedite transition planning, and collaborate with financial counselors and attending physicians. Care Transition Formulation: Develop transition plans in collaboration with patients, families, and healthcare teams, addressing complex clinical and social situations efficiently. Documentation and Compliance: Maintain current knowledge of regulatory requirements, consistently document in EHR, and ensure compliance with state and federal regulations. Operational Contribution: Identify and implement new systems and practices to improve patient/provider experiences and resource utilization. Communication and Collaboration: Foster positive relationships with healthcare teams and community agencies, utilizing effective communication skills to promote customer service.
