Pharmacist Position at VA Health Care System

We are seeking experienced Pharmacists for the VA Health Care System. This position includes both Inpatient and Outpatient Pharmacy roles, supporting Veterans by providing comprehensive pharmaceutical services. The pharmacist will ensure medication safety, conduct patient consultations, and work closely with healthcare teams to optimize patient care.

Key Responsibilities:

• Verify and dispense medications for inpatient and outpatient use.
• Conduct medication therapy management and provide patient counseling.
• Manage controlled substances and ensure compliance with VA regulations.
• Collaborate with healthcare providers to develop and adjust medication plans.
• Oversee medication preparation, compounding, and administration protocols.
• Document all pharmaceutical activities in VISTA/CPRS electronic medical records.
• Provide drug information and training to pharmacy staff and healthcare teams.
• Maintain compliance with all VA policies, state and federal laws, and pharmacy best practices.

Schedule: 40-hour workweek with 8-hour shifts, including day, evening, night, weekend, and holiday rotations as needed.

Requirements:

• Licensure: Active, unrestricted Pharmacist License in any U.S. State, Territory, or the District of Columbia.
• Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited college with a BS in Pharmacy or Doctor of Pharmacy (Pharm.D.) degree.
• Experience: Minimum 3 years of experience in an inpatient or outpatient pharmacy setting (VA experience preferred).
• Technical Proficiency: Knowledge of VISTA/CPRS electronic medical record systems. Experience with medication reconciliation, sterile and non-sterile compounding, and controlled substance handling.
• Other Requirements: U.S. Citizenship or Green Card Holder.
