Position Summary:
Provides and administers Occupational Therapy services to residents/clients, including comprehensive evaluation, therapeutic intervention, and development/execution of treatment plans. Operates in accordance with physicians' directives and adheres to established occupational therapy standards, facility policies, and applicable state regulations. Exercises supervisory responsibility over Occupational Therapy Assistants and Aides. Services are provided in accordance with the mission of Guardian Angels.

Job Duties:

• Optimize the physical abilities and skills of residents/clients by performing or supervising therapy staff in the following activities: initiating referrals; evaluating and treating appropriate patients; complying with Minimum Data Set procedures; effectively participating in the care planning process; appropriately documenting progress toward identified objectives and communicating resident/client status and needs to the resident/client, the resident/client family, the staff and other professionals as appropriate.
• Provides/recommends/fabricates adaptive devices, orthotics, or prosthetics. Trains patient, family, and appropriate staff in the use of the above application.
• Participates in discharge planning.
• Supervises students and volunteers.
• Refers the resident/client for treatment to Certified Occupational Therapy Assistants (COTA) by completing evaluation and initial plan of treatment and communicating necessary and sufficient information.
• Adheres to facility policies and procedures regarding documentation of occupational therapy services.
• Attend in-service training and faculty meetings as required.
• Provides in-service education to the members of the resident/client care team and to members of the community.
• Assumes responsibility for ongoing continuing education and professional development.
• Submits billing, payroll, and expense documentation in an accurate and timely manner.
• Assists in the collection of statistics as directed.
• Supervises and directs COTA in the implementation of the treatment plan.
• Supervises Rehabilitation aides.
• Reviews the quality and appropriateness of the total services delivered and of individual occupational therapy programs for effectiveness and efficiency, using predetermined criteria.
• Participates in resident/client, family, and staff education.
• Confers formally and informally with other team members in coordinating the total rehabilitation program of the patient.
• Conducts screenings of residents/clients at regular intervals to determine the need for intervention/ treatment.
• Develops treatment plans by identifying a problem list, including long and short-term goals and methods to achieve identified goals.
• Coordinates treatment plan with resident/client, family, and other team members.
• Monitors patient's response to intervention and modifies treatment as indicated to attain goals in accordance with the physician's orders.
• Implements and conducts treatment as outlined in the treatment plan.
• Establishes individual patient schedules in conjunction with other team members.
• Adapts treatment to meet the individual needs of the patient.
• Develops appropriate home or community programming to maintain and enhance the performance of the resident/client in his/her own environment.
• Documents results of patient's assessment, treatment, follow-up, and termination of services.
• Provides treatment in accordance with established standards of practice, department procedures, and efficiency standards.
• Consistently demonstrates sound judgment in the evaluation, planning, implementation, and follow-up of the patient therapy programs.
• Maintains positive relationships and rapport with coworkers, patients, family members, and facility personnel.
• Assists with and participates in Continuous Quality Improvement Program as required.
• Maintains a safe and clean work area and adheres to facility/corporate safety standards.
• To assist in providing a dignified, respectful, resident-centered environment for residents.
• Complies with all Human Resources policies and policies set forth by the organization.
• Follows universal precautions, facility infection control guidelines, sanitation, and safety standards of practice in all work and activities.
• All duties will be performed in accordance with current standards, guidelines, state, and federal regulations.
• Uphold confidential information as required by Guardian Angels policy and procedure and as related to HIPAA.

• Performs all other duties as assigned.

Qualifications:

Education/Experience:

• Bachelor of Science in Occupational Therapy (Masters or Doctorate as required per graduation date of employee) from an accredited program.
• Completion of the National Certification Examination for Registered Occupational Therapists and licensed in the state of MN.

Experience:

• One to three years prior experience in a rehabilitation settings such as acute care, inpatient rehab, outpatient clinics, SNFS, long-term care, assisted living, memory care, or home health.
• Familiarity with fall prevention strategies and mobility training for seniors.
• The ability to use therapeutic techniques to help clients build or restore skills.
• Knowledge of State and Federal Regulations.
• Excellent verbal and written (read, write) communication skills.
• Self-motivated, ability to prioritize.
• Proficient with Microsoft Office 365 required. Net Health software experience required. Familiarity with other healthcare software, such as PointClickCare (PCC), is a plus.
• Maintains good attendance and punctuality.
• Ability to work varying hours, including weekends and holidays.
