Hospice RN Case Manager

Suncoast Hospice, a part of Empath Health, is seeking a caring and motivated RN Case Manager to work with us in the South Hillsborough area (Valrico, Riverview & Lithia) while providing exceptional care on the following schedule: Monday-Friday 8a-5p.

What you'll do:

• Under the direction of a licensed physician, manage discomfort and provide symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Provide initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial, and environmental needs and activities of daily living.
• In collaboration with the other members of the Interdisciplinary Group (IDG), develop, implement, and update the individualized plan of care, initiate appropriate preventive and rehabilitative procedures, and initiate referrals to other services; request complimentary services, as needed.
• Educate patient/resident, family, and caregivers regarding disease process and decline, palliative interventions, care giving, dying process, and safety practices.

What you'll need:

• Graduate of approved nursing program and current unrestricted Florida RN license.
• Valid and current CPR certification required.
• Minimum one year experience in an acute-care hospital environment providing direct patient care; previous experience as a Hospice RN preferred.
• Bilingual preferred.
• CHPN certification is strongly encouraged within three years of employment.
• BSN or MSN preferred.
