Pay: $34.13 per hour

Schedule: Monday–Friday, 8:00 AM–5:00 PM EST

Location: Work from home + 50–75% travel for face-to-face member visits in Miami

Field Health Care Case Manager Summary: Field Health Care Case Management role supporting Medicaid Long Term Care/Comprehensive Program members. Responsible for telephonic and face-to-face assessment, care planning, care coordination, and documentation, collaborating with providers and using care management tools to support medically complex populations.

Field Health Care Case Manager Requirements:

• Fluent bilingual Spanish/English (read, speak, write) — required
• 1+ year case management experience — required
• Bachelor’s degree required (social work or related field preferred); no nurses
• Comfortable with 50–75% travel for member visits (home/assisted living/nursing homes)
• Proficient with Microsoft Word, Excel, Outlook
• Strong communication skills; able to multitask and work in a fast-paced environment
• Long-term care experience (preferred)
• Case management certificate (preferred)
