What you’ll bring: Active RN licensure in Texas or LMSW (LCSW preferred) Case Manager Certification (CCM or equivalent) required 5+ years in utilization management, case management, discharge planning, or a related cost/quality program 3+ years of hospital-based nursing or social work experience 3+ years of demonstrated leadership experience Compensation: $108,000 - $130,000 annually, commensurate with experience **2 openings - 1 for Trauma and another for ED Case management at the hospital level is where care continuity gets won or lost.
