About the Role

Responsibilities

Provide direct patient care, including administering medications, taking vital signs, and assisting with daily living activities

Develop and implement personalized care plans in collaboration with our healthcare team

Communicate effectively with our patients, families, and healthcare team members to ensure seamless care coordination

Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction

Maintains accurate and timely documentation of patient records
