- $75K-$85K base salary (depending on experience and license level)

- Evaluate, assess, and support acute + chronic social determinants of health for the Mineola community
- Connect patients to local community resources, housing, and financial assistance
- Create care coordination plans and coordinate follow-up care
- Educate patients and families on mental wellness and systemic healthcare navigation
- Document visits in EMR using technology-driven tools
- Master's Degree in Social Work (MSW) from an accredited institution
- Active New York State license (LMSW or LCSW)
- Board certification or state-approved licensure in good standing
- EMR experience and strong HIPAA knowledge
- Excellent communication, active listening, and crisis-de-escalation skills
- Preferred: 1+ year healthcare/outpatient clinic experience; bilingual English/Spanish
- New grads are welcome (must hold active NYS licensure)
