Qualifications

The contractor shall provide nurses with the following qualifications: Registered Nurses (Rns). Contract Rns, including Specialty Registered Nurses (those with experience in Intensive Care, Medical-Surgical Units, Telemetry and Cardiac Monitoring and Emergency Room) shall have:

• Graduation from a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (Acen) or The Commission on Collegiate Nursing Education (Ccne).
• A current license from the Ohio Board of Nursing or a current multi-state license from a Nurse Licensure Compact (Nlc) state.
• Current certification in Cpr by an American Health Association Vendor.
• Current Basic Life Support certification. If assigned to Intensive Care Unit/Step Down or Emergency Department, contract nurse/Rn shall also have current Acs certification.
• A current Cv which identifies the provider's education and professional qualifications commensurate with the position or specialty for which they are being offered to perform. Specialty certification preferred.
• A minimum of two (2) years direct patient care experience within the last three (3) years. Va experience is desirable

Technical Proficiency

Contractor's nurses/Rns shall be technically proficient in the skills necessary to fulfill the government's requirements, including the ability to speak, understand, read and write English fluently. Contractor shall provide verifiable evidence of all educational and training experiences including any gaps in educational history for all contract personnel(s).

Roles And Responsibilities

Scope Of Care: General. The Rn assigned to these areas provide direct and indirect nursing care, by contributing to the patient's assessment, determining priorities, identifying nursing measures and therapeutic objectives and evaluating outcomes. The position requires independent judgment and nursing actions, flexibility, organizational skills, and the ability to utilize an interdisciplinary approach in the provision of nursing care. Organizational Relationship. The Rn is subject to the professional direction of the Va Nurse Manager or designee during the workweek. This is NOT an employer employee relationship. On evenings, nights, holidays and weekends, the Rn reports through the Charge Nurse to the Nursing Supervisor/Nursing Officer of the Day (Nod). All contracted Rns are required to float among various units within the service area which they are hired, based on operational needs. Floating assignments will be determined by the Va Nurse Manager or designee, Charge Nurse, Nursing Supervisor/Nod to maintain adequate staffing levels and continuity of care.

Community Living Center Inpatient (Clc): Clc Short Stay Skilled Inpatient is designed to provide short-term, post-acute skilled nursing and rehabilitation services to patients who require intensive clinical care and therapy following hospitalization, surgery, or acute illness. The Clc provides Veterans with a homelike setting with private or semi-private rooms, therapy gyms, and adaptive equipment. This model emphasizes rehabilitation, recovery, and safe transition to home or community setting, rather than long-term custodial care. Patients are medically stable but require skilled nursing interventions, intensive rehabilitation therapies, monitoring and support for chronic conditions during recovery. Core services include:

• 24/7 Skilled Nursing Care: Medication administration, wound care, Iv therapy, pain management.
• Rehabilitation Therapies: Physical, occupational, and speech therapy integrated into daily routines.
• Interdisciplinary Care Planning: Regular team meetings to review progress and adjust care plans.
• Discharge Planning: Coordination with outpatient services, home health, and community resources.

Rns assigned to work in the Clc will be responsible for patient assessments, medication administration, Iv therapy, drawing labs, central and peripheral line Iv medications, tube feedings, foley catheters, wound care, trach care, assistance with activities of daily living, interdisciplinary team coordination, admissions, discharges, internal and external transfers, delegating to assistive personnel, electronic medical record documentation, patient transport, and emergency response procedures. All Rns assigned to Clc inpatient under this contract will be required to float between units based on staffing needs.

Coral Cove (Ct2 ) is a 37-bed short stay rehab neighborhood. The registered nurse collaborates with an interdisciplinary team to provide comprehensive care to Veterans that are completing rehab with the goal of returning to their prior functional status after an acute medical stay. Admiral's Point (3b ) is a 14 bed post-acute neighborhood with a respiratory focus. The registered nurse collaborates with an interdisciplinary team to provide comprehensive care to the Veterans that require more respiratory focused care then what can be supported in the home environment. Waterside Way (Ct3 ) & Emerald Woods (Ct4) are both 37 bed short stay rehab and skilled neighborhoods. The registered nurse utilizes the nursing process to provide comprehensive care to Veterans that are admitted for rehab, Iv antibiotic management, and simple to complex wound care. The nurse will identify barriers to rehab or treatment plan and will collaborate with an interdisciplinary team to resolve these issues and promote a safe discharge to the Veterans' preferred location. Soaring Heights (Ct5 ) is a 37-bed secure cognitive impairment/dementia care neighborhood. The registered nurse provides comprehensive holistic care to veterans that are admitted to the floor due to respite, placement needs, short term rehab or in need of medical care that cannot be safely given at home. The nurse will collaborate with the interdisciplinary team to facilitate a safe and appropriate discharge. Blind Rehab (Wcta ) is a 15 Bed certified rehabilitation center for Veterans with various degrees of visual impairment. The registered nurse works with the interdisciplinary team to work with the Veteran while they receive several weeks of therapy with the goal of returning home to independent living. Heroes Harbor Hospice is a 12 bed neighborhood that provides services to Veterans enrolled in the Va Neohs who have been diagnosed with a life-limiting illness for which curative treatment is no longer appropriate, that would benefit from an inpatient interdisciplinary team, providing patient centered care with the goal to achieve the best quality of life through relief of suffering, control of symptoms and maintenance of functional capacity while remaining sensitive to personal, cultural and religious values, beliefs and practices. This also includes bereavement care for the Veteran's family.

Hours Of Operation: Va Business Hours : Va business hours/hours of operations are twenty-four hours a day, seven days per week (24/7), including holidays. Work Schedule : Shifts, tours of duty, and hours may vary depending on the unit. Contract nurses/Rns shall be available to work on days, nights, evenings, weekends, and Holidays. Shifts are primarily 12-hours, with some units offering 8-hour and 10-hour shifts. The Government reserves the right to schedule or reschedule the duty hours to meet staffing and patient care needs. The Government shall provide twenty-four (24) hour notice of any schedule changes. Contractor's nursing staff shall be present at the facility and shall be performing the required services to receive payment for services.
