LPN Nurse Care Manager

Licensed Practical Nurse (LPN)

Seeking a Licensed Practical Nurse (LPN) to support children and families in a community-based Preventive Services Program. This role focuses on health care coordination, family education, home visits, and connecting families with medical and community resources.

Responsibilities

• Coordinate healthcare services for children and families
• Conduct home visits and provide health education and support
• Review medical reports and coordinate follow-up care
• Assist families in accessing healthcare and community resources
• Collaborate with social workers, physicians, schools, and community providers
• Monitor children's healthcare needs and maintain documentation
• Accompany families to appointments as needed
• Support health monitoring and care planning for high-risk families

Requirements

• Current New York State LPN License
• Experience in community-based nursing, home care, care coordination, behavioral health, or pediatrics preferred
• Strong communication and organizational skills
• Ability to travel locally for home visits and appointments

This position includes field visits and direct interaction with children and families in the community.
