Job Description
Case Manager - Day Shift Assignment Duration This is a 13-week contract position. Schedule Day shift, 5x8-hour shifts from 08:00 to 16:30. Job Description The Case Manager is responsible for coordinating patient care to ensure appropriate and efficient delivery of services. This role involves assessing patient needs, developing treatment plans, and working closely with multidisciplinary teams and providers to facilitate care transitions across various healthcare settings. Responsibilities
- Conduct comprehensive assessments to determine patient needs and eligibility for services.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Arrange for necessary medical equipment, transport, and other services as needed.
- Participate in family meetings and care coordination discussions.
- Maintain accurate and timely documentation of patient interactions and care plans using Epic.
- Ensure compliance with regulations and standards in care delivery.
- Minimum of 2 years of case management experience.
- Experience in acute hospital settings; LTAC and trauma experience are preferred.
- Current RN license or relevant certification as required.
- Knowledge of CMS guidelines, HIPAA regulations, and care coordination principles.
- Competency with computerized documentation systems (Epic required).
