Role & Responsibilities

• Compassionate End-of-Life Care: Provide sensitive resident-centric daily personal care to patients in a long-term care setting ensuring comfort dignity and peace.
• Patient Hygiene & Comfort: Assist with personal hygiene grooming bathing light nutrition and repositioning/safe transfers. Measure and record vital signs.
• Family Support & Communication: Serve as a comforting professional presence reporting any clinical changes or urgent needs promptly to the hospice nursing supervisor.
• Environment Maintenance: Assist with maintaining a clean calm and supportive home environment for the patient and their visiting loved ones.
• Technology Proficiency: Demonstrated ability to navigate MatrixCare efficiently with your cellphone ensuring timely and accurate documentation of patient care timesheets and travel time.
• Ability to Travel (when needed): Capable of independent travel to various local client residences when needed and maintaining detailed and precise records of travel time and destinations.

Requirements

• Must possess an active Pennsylvania CNA certification
• Minimum 6 months of hospice home health skilled-nursing or related clinical experience
• Highly punctual reliable and trustworthy
• A deeply caring empathetic and resilient attitude suited for home hospice environments
• A reliable vehicle and Drivers License
