Job Summary
Physical Therapist Home Health (PT HH) in the San Ramon area. This position involves providing in-home therapeutic services to patients within designated territories, with the possibility of travel outside the service areas. The role offers flexibility in schedule and patient care responsibilities, supporting the delivery of quality outpatient home health services.

Job Details

• Schedule: Days shift, 12 hours per shift, minimum of 36 hours per week
• Required Qualifications: Current California Physical Therapist (PT) license, BLS certification, valid drivers license, auto insurance, auto registration, TB test (annual), MMR, Varicella, Hep B, Tdap (valid within 10 years), annual flu shot, drug screen, physical exam, 1 year of relevant experience
• Preferred Qualifications: Experience in home health settings is preferred
• Additional Information: Position involves traveling outside the designated service areas and working weekends as required. Guarantee policies vary depending on the assignment specifics and patient caseload. All requested time off (RTO) must be disclosed at the time of submission; post-acceptance RTO requests may not be approved. The position starts on 08/24/2026.
