Job Overview South Shore VNA is looking for experienced Physical Therapists to provide home‑based rehabilitation services across the South Shore region. Key Information Facility: 30 Reservoir Park Drive, Rockland, MA. Department: SSH VNA Team 1. Schedule: Mon‑Fri 8‑4:30 pm; 1 weekend shift/month; 1 holiday/year. Compensation Pay range: $85,716.80 – $115,356.80 per year. Responsibilities Evaluate and treat patients with a variety of sports, orthopedic and physical disorders. Work cooperatively with an interdisciplinary team to achieve care goals. Organize and direct treatment activities of patient assistants, rehabilitation aides and/or co‑op students. Document and bill services following department standards and insurance parameters. Maintain patient safety, infection control, and professional documentation practices. Use evidence‑based practice to plan, direct, and implement patient care. Educate patients and families, provide home exercise programs. Discharge patients when skilled therapy is no longer appropriate. Support South Shore Health policies and values. Essential Functions Provide comprehensive evaluations, re‑evaluations, and treatments through discharge. Practice within Massachusetts legal boundaries and professional guidelines. Direct and delegate tasks to licensed and non‑licensed staff. Maintain infection control and patient safety practices. Establish realistic goals and develop care plans. Implement treatment interventions based on goals. Provide documentation and charge capture accurately. Strive to meet 100% productivity standards. Utilize technology and software for job performance. Communicate effectively with staff, patients, and families. Maintain professional growth and supervision of clinical students. Demonstrate professionalism, composure, and critical thinking. Adapt to scheduling needs and be flexible. Non‑Essential Functions Identify and assume duties related to departmental development. Perform other duties as assigned. Qualifications BS, MS or DPT from an accredited physical therapy school. Previous clinical or rehabilitation experience preferred; orthopedic rehabilitation experience desirable. BLS certification. Licensed Physical Therapist in Massachusetts. License & Registration Physical Therapist – Massachusetts Department of Public Health. Basic Life Support certification – American Heart Association.
