We are seeking a dependable and compassionate Per Diem Certified Nursing Assistant (CNA) to provide hands-on care and support to residents. The CNA will follow assigned plans of care, assist residents with activities of daily living, and communicate important resident information to the licensed nursing team.

The ideal candidate is attentive, reliable, and committed to providing quality resident care while working effectively as part of a healthcare team.

Responsibilities

• Follow the resident assignment sheet and individual plan of care
• Assist residents with activities of daily living, including meals, hygiene, and other personal care needs
• Listen carefully to shift report and prioritize resident care appropriately
• Communicate relevant resident care information to the licensed nurse throughout the shift
• Notify the licensed nurse when leaving the unit and at the end of the shift
• Communicate important information with coworkers to help maintain continuity and quality of resident care
• Complete all required ADL documentation by the end of the scheduled shift
• Follow facility policies, procedures, and resident care standards

Requirements

• Minimum 1 year of CNA experience
• Valid New York State Nurse Aide certification
• Current physical examination and immunization records
• Ability to communicate effectively with residents, nurses, and other members of the healthcare team
• Reliable attendance and ability to follow assigned schedules

Dress Code

• Scrubs are required while working in the facility.
