Responsibilities
- Conduct comprehensive speech, language, and swallowing assessments for residents in the facility.
- Develop and implement individualized treatment plans; monitor and document progress in the medical record.
- Provide bedside swallow evaluations and manage dysphagia care with appropriate safety strategies.
- Collaborate with an interdisciplinary team (nursing, therapy, dietary, social work) to optimize communication and feeding outcomes.
- Reassess residents regularly, adjust goals as needed, and educate patients and families on therapy approaches and home exercise programs.
- Support discharge planning and transitional care to align with resident needs and placement goals.

Requirements
- Active license to practice Speech-Language Pathology in Connecticut (or eligible for licensure in CT)
- Master’s degree in Speech-Language Pathology from an accredited program
- Experience in acute care and/or long-term care settings preferred
- Ability to work effectively within a multidisciplinary team
- Strong clinical documentation and communication skills
