Schedule: Variable - 7 days per week departmental coverage with a generally scheduled 5-day work week. Day shift, including possible weekend days. Job Description The Rehabilitation Therapist will provide comprehensive rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). Responsibilities include conducting evaluations, developing and implementing individualized treatment plans, and collaborating with other healthcare professionals to enhance the patients' quality of life. Responsibilities Perform evaluations, reassessments, treatments, and other clinical services in accordance with licensure and scope of practice. Develop, implement, and carry out individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary teams. Implement evidence-based therapeutic interventions to maximize function, independence, communication, mobility, swallowing, and quality of life. Educate residents, families, and caregivers regarding treatment plans and patient progress. Complete accurate, timely, and compliant documentation of patient progress, treatment changes, and discharges per facility and regulatory requirements. Supervise therapy assistants as applicable to the role. Be available to work during Hawaii State and Federal Holidays as necessary based on facility operations. Required Experience / Certifications / Licensure Minimum of two (2) years of experience in a Skilled Nursing Facility (SNF) or Intermediate Care Facility (ICF) setting preferred. Current Basic Life Support (BLS) certification. Current State of Hawaii license in good standing appropriate to the discipline is required by start date. Why ARMStaffing?
