Position Overview
We are seeking an experienced Home Health Registered Nurse to deliver high-quality, patient-centered care in a field-based setting. This role focuses on direct patient care, OASIS assessments, and care coordination, ensuring patients receive effective treatment and education in the comfort of their homes. The ideal candidate is skilled in home health practices, confident working independently, and experienced with OASIS and HomeCare Homebase documentation. Schedule & Work Details
Location: Coverage in Christian and Trigg Counties
Shift: Monday through Friday, 8:00 AM to 4:30 PM
Weekly Hours: 32-hour guarantee
On-Call/Weekend Requirements: Rotational weekday and weekend on-call
Mileage Reimbursement: Yes, reimbursed between patient visits from first to last patient
Pay Rate: $51 to $53 per hour Key Responsibilities

- Provide direct patient care, including skilled nursing services in the home setting
- Perform comprehensive patient assessments and develop individualized Plans of Care (POC)
- Complete OASIS documentation including Start of Care, Resumption of Care, Recertifications, and Discharges
- Deliver patient and caregiver education on disease processes, medications, and care techniques
- Perform clinical procedures including wound care, IV therapy, sterile dressing changes, and venipuncture
- Coordinate care with physicians and interdisciplinary team members to ensure optimal patient outcomes
- Maintain accurate, timely documentation in HomeCare Homebase (HCHB) and ensure compliance with regulatory standards
- Participate in on-call rotation to support patient needs outside of regular hours
- Active Registered Nurse (RN) license
- Prior home health experience
- Proficiency with OASIS documentation
- Experience with HomeCare Homebase (HCHB)
- Strong clinical assessment and patient care skills
- Proven experience in home health nursing
- Strong OASIS knowledge and documentation accuracy
- Experience developing and managing Plans of Care (POC)
- Excellent time management and ability to work independently
