Key Responsibilities Highlights Accountability for outcomes: Demonstrate accountability for outcomes, strong clinical care, and cost‑effectiveness for up to 450 patients, building trusting relationships and helping them change behaviors. Coaching for health: Act as a health coach, helping patients set short and long‑term health goals, partnering to achieve them, and frequently following up on goals to improve health. Simplifying for action: Simplify and prioritize appropriately so that behavior change is actionable for patients and for leading care teams toward performance goals. Essential Duties & Responsibilities The PCP independently provides care for patients with acute and chronic illnesses encountered in older adults. The PCP takes full accountability for patient care and outcomes, seeks consultation from specialists when needed, and remains involved in detailed care. The PCP engages with hospitalists for patients hospitalized, regardless of hospitalist affiliation. Responsibilities include assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic needs. The PCP leads a care team of care promoter (medical assistant), care facilitator, and care coordinator for office‑bound patients; for those unable to come to office—in hospital, SNF, LTC, or homebound—the PCP engages the transitional care team, case managers, and other resources. The PCP has an active role in center management, covers for other providers as needed, participates in recruiting patients and additional providers, supervises and collaborates with an Advanced Practice Practitioner (APP) while remaining accountable for the APP’s actions, and performs additional duties as assigned by management. Knowledge, Skills & Abilities Availability and accessibility: Make themselves available to patients through walk‑in visits and timely response to calls and messages. Service orientation: Provide care that they would want for a family member or themselves to each patient at every interaction. Evidence‑based medicine: Stay updated on evidence‑based medicine and recognize the impact of lifestyle and nutrition on patient outcomes, remaining current on clinical, nutritional, and lifestyle interventions. Physician leadership: Continuously develop and improve leadership skills for the benefit of patients, team, center, and company. Quality focus: Work with care teams to achieve high‑quality outcomes and quality measures, while continually improving medical practice and center operations. Influence: Competently and compassionately influence patients, teams, and themselves to achieve optimal outcomes. Self‑care: Maintain physical, mental, social, and spiritual well‑being to provide the best care to patients. Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other word‑processing, spreadsheet, database, e‑mail, and presentation software used in the company. Travel flexibility: Willingness and ability to travel locally as needed, nationally for initial training (2 weeks) and occasionally regionally or nationally for recruiting or training. Fluency in English, verbal and written; additional language fluency may be required depending on the center. Independent judgment: Use and exercise independent judgment in all aspects of patient care and administrative responsibilities.
