- Conduct thorough evaluations of patients across both inpatient and outpatient scenarios, crafting personalized treatment plans tailored to their needs.
- Implement effective physical therapy interventions focused on enhancing mobility, strength, balance, and overall functionality.
- Engage collaboratively with an interdisciplinary team, including nurses, physicians, case managers, and other rehabilitation professionals, to maximize patient outcomes.
- Provide education for patients and their families on therapeutic objectives, discharge strategies, and appropriate home exercises.
- Meticulously document evaluations, treatment processes, and progression in accordance with regulatory standards and facility guidelines.
- Take part in care team meetings and interdisciplinary conferences to ensure seamless coordinated care for our patients.
- A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution is required.
- A current state licensure as a Physical Therapist is mandatory.
- CPR certification is preferred.
- Specialized certifications such as in geriatrics or neurology are advantageous but not necessary.
- Proven experience in a relevant setting is strongly preferred.
- Capability to manage patients dealing with complex medical and mobility challenges.
- Excellent communication skills and the ability to work effectively within a team.
- Familiarity with digital charting and clinical documentation systems is essential.
