Description
  • Conduct thorough evaluations of patients across both inpatient and outpatient scenarios, crafting personalized treatment plans tailored to their needs.
  • Implement effective physical therapy interventions focused on enhancing mobility, strength, balance, and overall functionality.
  • Engage collaboratively with an interdisciplinary team, including nurses, physicians, case managers, and other rehabilitation professionals, to maximize patient outcomes.
  • Provide education for patients and their families on therapeutic objectives, discharge strategies, and appropriate home exercises.
  • Meticulously document evaluations, treatment processes, and progression in accordance with regulatory standards and facility guidelines.
  • Take part in care team meetings and interdisciplinary conferences to ensure seamless coordinated care for our patients.
Education
  • A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution is required.
Certifications
  • A current state licensure as a Physical Therapist is mandatory.
  • CPR certification is preferred.
  • Specialized certifications such as in geriatrics or neurology are advantageous but not necessary.
Skills
  • Proven experience in a relevant setting is strongly preferred.
  • Capability to manage patients dealing with complex medical and mobility challenges.
