Community Health Worker

Oak Street Health seeks a Community Health Worker to support Medicare patients in underserved communities. In this role, you will build trusted relationships, conduct outreach, and connect patients with primary care, social services, and preventive programs. You’ll help coordinate appointments, provide health education, and address barriers like transportation or housing. Working closely with clinicians and care teams, you’ll track patient progress, maintain accurate documentation, and promote adherence to care plans to improve outcomes in a patient-centered, inclusive setting.
