DISCHARGE PLANNER

Windsor Laurelwood is seeking a Discharge Planner. Responsibilities In this role, you will coordinate discharge plans for psychiatric inpatient and partial hospitalization patients, connect them with aftercare resources, and collaborate with an interdisciplinary team to ensure a smooth transition from care. Coordinate and lead discharge planning, including scheduling follow-up appointments and communicating with aftercare providers. Collaborate with referral sources such as SASS, DCFS, and other agencies. Build therapeutic relationships with patients and provide ongoing support during their stay. Educate patients and families on mental health conditions and treatment options. Participate in interdisciplinary team meetings and clinical staffing. Advocate for patients and contribute to program development initiatives. Perform other duties as assigned. Qualifications Knowledge of mental health and/or chemical dependency assessments. Strong written and verbal communication skills. Ability to work independently and as part of a multidisciplinary team. Experience providing therapeutic and group interventions (Cognitive Behavioral Therapy focus preferred). Basic crisis intervention skills. Ability to manage competing priorities in a fast‑paced environment. Understanding of patient rights, abuse reporting laws, and emergency regulations. Demonstrated patience, professionalism, and resourcefulness. Education & Experience Bachelor’s degree required (Master’s preferred). 0–1 year of experience required; 1–3 years preferred in behavioral health or related field. Benefits Tuition reimbursement.
