Schedule / Job Type

Contract | Full-Time | Onsite | Day Shift | Monday through Friday, 7:00 AM – 3:00 PM or 8:00 AM – 4:00 PM (flexible) | 32 guaranteed hours per week

Key Responsibilities

• Provide occupational therapy services to patients in a Skilled Nursing Facility (SNF) setting
• Deliver occupational therapy services to patients in a Home Health setting
• Meet productivity expectations of 87% in the SNF setting and 95% in the Home Health setting
• Utilize Net Health and Home Care Home Base software for documentation and patient management
• Collaborate with an onsite Therapy Supervisor to support patient care goals

Required Qualifications

• Active Certified Occupational Therapy Assistant (COTA) license in the state of Minnesota
• Ability to work across SNF and Home Health settings
• Proficiency with or ability to learn Net Health and Home Care Home Base software
• Ability to meet setting-specific productivity standards

Preferred Experience / Skills

• Prior experience in a Skilled Nursing Facility (SNF) environment
• Prior experience in a Home Health setting
• New graduates are welcome to apply
