Physical Therapist- Healthcare

Job Summary The Physical Therapist is responsible for providing and supervising primary care to approximately 25-35 patients in a home setting. This includes age‑appropriate interventions that meet the physical, emotional, developmental, psychosocial, and educational needs of each patient. The Physical Therapist coordinates all rehabilitative disciplines throughout the episode of care. Responsibilities Communicate with all team members (RNs, LPNs, therapists, social workers, Home Health Aides, the Physician, patient, family) to ensure the plan of care is appropriately developed, implemented, and goals are met. Supervise PTAs and Home Health Aides throughout the patient plan of care. Assist in developing and implementing processes to improve patient and agency outcomes. Assess, coordinate with other disciplines, and manage patients’ total needs, following physician orders and using the physical therapy process. Develop patients’ plan of care and coordinate with other disciplines. Manage patient from admission through recertification to discharge and schedule visits per physician order. Perform routine and complex therapy procedures to meet the patient’s needs. Revise the plan of care based on patient needs within physician orders. Develop and supervise the care plan provided to the Home Health Aides. Review plan of care for patients with the Manager Home Health and other team members and support services. Maintain communication with physician reporting any changes that may affect the patient’s recovery. Participate in orientation and education of new employees as appropriate. Attend continuing education and in‑services as directed. Remain current in clinical knowledge for the care of the home health patient. Communicate patient status changes and lab results to the physician as appropriate. Check voicemail box and staff messaging at least once daily. Demonstrate knowledge and appropriate application of CMS/State and Joint Commission regulatory guidelines. Accurately complete required OASIS and visit documentation in the patient’s EMR within established timeframes. Participate in department performance improvement activities (e.g., chart review, committees). Identify methods for improving department performance/care. Assist in implementing performance improvement opportunities. Meet established productivity standards, weekend and call rotation as required. Perform other duties as assigned by Home Health Leadership. Patient / Caregiver Education Participate in the orientation and education of new employees and remain current in clinical knowledge for the care of the home health patient. Regulatory Guidelines Demonstrate knowledge and appropriate application of CMS/State and Joint Commission regulatory guidelines. Accurately complete required OASIS and visit documentation in the patient’s EMR within established timeframes. Performance Improvement Participate in department performance improvement activities and identify methods for improving department performance/care. Other Responsibilities Meet established productivity standards, weekend and call rotation as required. Requirements Graduate of an accredited school of physical therapy. Private vehicle insurance. Additional license(s) and certification(s) as required. Minimum 1 year of full‑time Physical Therapy experience (Home Health experience preferred). Strong critical care assessment and rehabilitation skills. Proficiency with Windows‑based computers, home health patient software, email, internet, and computer‑based learning modules.
