Case Manager

Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Position : Case Manager (Registered Nurse or Social Service Professional) Key Responsibilities : Plan, organize, and direct hospice care, emphasizing community health education and patient comfort. Provide intake, assessment, and case management services for clients, ensuring ongoing eligibility and monitoring all aspects of care. Deliver comprehensive case management to hospitalized patients to improve quality of life and maintain functional ability within the community. Ensure compliance with state and federal regulations for special education or other applicable programs. Coordinate and collaborate with interdisciplinary teams, family members, and community resources to support client-centered care. Qualifications : Baccalaureate degree in Social Work, Psychology, Sociology, or a related field with at least 2 years of direct social service experience. Associate degree in Human Services or a related field with at least 4 years of direct social service experience. Registered Nurse (RN) with an unrestricted license, experienced in nursing and community health education. Strong knowledge of case management principles, crisis intervention, and client advocacy. Excellent communication, organizational, and interpersonal skills. Compensation : Hourly range: $43.00 – $51.46 Annual salary range: $50,000 – $88,000 Employment Type : Full‑time, on‑site.
