Maryville is seeking an experienced full-time SocialServices Coordinator to join its well-respected team of dedicated health-care professionals. Assessing the resident's psychosocial and emotional needs. Effectively communicating information concerning a resident’s condition with other departments and government agencies. Ensuring that the medically related emotional andsocialneeds of the resident are met and maintained. Participating in the resident discharge planning, providing information to residents and families of programs available. Serving as a liaison with community agencies to ensure needed ancillary services are available and provided within requested timeframes. Making referrals and documenting outcomes. A strong commitment to maintaining the confidentiality of all resident care information in accordance with HIPAA Guidelines. Qualifications: Bachelor’s degreeis required; focused inSocialWork or other Healthcare related fields required. The ability to pass a background check is required of all Maryville Employees Recent experience in aSocialServices role in a nursing home/hospital setting or some otherSocialWorker role required. Ability to work in a fast-paced work environment. Excellent communication and customer service skills. Strong computer skills required due to this facility working solely in an Electronic Medical Records environment. Bachelor’s degreeis required; focused inSocialWork or other Healthcare related fields required. The ability to pass a background check is required of all Maryville Employees Recent experience in aSocialServices role in a nursing home/hospital setting or some otherSocialWorker role required. Ability to work in a fast-paced work environment. Excellent communication and customer service skills. Strong computer skills required due to this facility working solely in an Electronic Medical Records environment. Physical requirements: Must be able to lift 25 pounds at times. Required to stand or walk for prolonged periods. Ability to lift, push, pull or move at least 50 pounds independently. Requires regular bending, stooping, reaching, and kneeling.
