Description:

As aHospice Aide/Certified Nursing Assistant (CNA),you are one of the most vital componentsofthe Interdisciplinary Team! You are the heart, the eyes, and the key communicator of your patients needs. Your role is to follow a carefully planned out, personalized Care Plan for each patient you are assignedand to report and work closely with the RN Case Manager to ensure the care of the patient is completed and expectationsexceeded.You will effectively interact with all members of the Interdisciplinary Teamas well as with your patients and their loved oneswhile maintaining the highest standards of care and professional nursing clinical competency.

MinimumRequirements:

CurrentCNALicense in the State of Georgia
Current CPR & First Aid Certification
1-2 years hospice experience (other candidates may be considered with experience in assisted living, home health, long term care)
Utmost compassion & integrity
Drivers license,insuranceand vehicle
Excellent Time Management Skills
The ideal candidate would live in or around the Trenton area
