Assist with activities of daily living. Encourages and fosters independence with the patients as indicated. Communicates with the medical staff on identified patient issues. Performs patient care assessments timely and accurately. Addresses patient care needs in a timely manner. Documents findings into the medical record accurately and timely. Ensures daily documentation is completed thoroughly and signed and dated according to protocol. Adheres to Human Resource policies regarding code of conduct and attendance. Follows Infection Control and Employee Health Guidelines to ensure safety of patients and staff. Actively works with the Service Development Department to facilitate admissions into the facility in a timely manner. Functions as a member of the multidisciplinary team and communicates identified patient goals, interventions, and assessments.
