Internal Medicine Physician Pod Leader - PACE - North Hollywood, California

- Primary Care Physician with focus on geriatric, community based, value based care
- Full-time employed position in an outpatient PACE model
- Average of approximately 8 patients per day with no hospital rounding
- Regular schedule Monday through Friday, 8 a.m. to 5 p.m., including some home visits
- Rotating phone only call a few nights or weekends per month
- Perform comprehensive in person assessments and lead the interdisciplinary team to create care plans
- Coordinate discharges and placement from acute, short term, and long term care settings
- Emphasis on interdisciplinary team based care for Medicare and Medi Cal eligible seniors
- Competitive base salary in the range of $275,246 to $330,595 with quarterly bonus eligibility
- Full benefits including medical, dental, vision, 401K with match, PTO, sick time, and paid holidays
- CME reimbursement, malpractice and tail coverage, and relocation assistance available
- Growth opportunities including potential advancement to medical director roles
- MD or DO with completion of an ACGME accredited residency and doctorate level degree
- Active unrestricted state medical license, state controlled substance registration, and DEA required; CPR and First Aid required within 180 days of hire
- Board certification preferred in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics with 2 to 5 years of primary care experience in geriatrics preferred
- Located in North Hollywood, California near attractions including the NoHo Arts District, Universal Studios Hollywood, and Griffith Park
- Approximately 12 miles (about 25 minutes depending on traffic) to downtown Los Angeles and 25 miles (about 40 to 50 minutes) to Santa Monica and Pacific Coast beaches
