Registered Nurse (RN), Care Manager, Part Time

Job Summary Pay Range: $35.92 - $53.88. A successful candidate's actual pay rate will be based on relevant experience, skills, training, certifications and education. Case Managers at HSHS ensure patients receive cost‑effective, high‑quality care by collaborating with patients, families, physicians, interdisciplinary teams and payor representatives. Responsibilities Ensure patients receive high‑quality care that is truly reflective of the services and care needed to heal. Collaborate with patients, families, physicians, interdisciplinary teams and payor representatives. Work to provide cost‑effective care in the most clinically appropriate setting. Qualifications 2 years of clinical experience. Associate’s Degree or Diploma in Nursing (Bachelor’s Degree preferred). Prior case management experience preferred. Quality/Process Improvement experience preferred. Certifications & Licenses Registered Nurse (RN) license in the state of practice. Wisconsin: RNs may practice with a valid multi‑state RN license as outlined in the Nurse Licensure Compact rules. Case Management certification (ACM, CCM, RN-BC) preferred. Basic Life Support Certification (BLS) required within 60 days of hire. Compensation Part‑time schedule: 16 hours/week – Every Saturday and Sunday; 8‑hour shifts.
