Who we are looking for Experienced LCSW passionate about providing counseling services that support an individual’s physical, mental, cognitive, and emotional adjustment through recovery. Provide therapeutic interventions and resource recommendations that address patient’s goals to achieve their personal, career, and independent living goals in the most integrated setting possible. Responsibilities Perform comprehensive evaluations, assessing the social, emotional and support needs of patients and their families. Set measurable objectives that are formulated in conjunction with the rehab team. Assist the physician and other health team members in understanding the significant social and emotional factors related to the patient’s health problems. Instruct and counsel patients and families in managing social and emotional responses related to illnesses. Participate in care coordination activities and act as a resource to other health team members in the identification and resolution of patient needs. Identify and assist the patient and/or family in utilizing appropriate community resources to achieve measurable objectives. Qualifications Master’s Degree in Clinical Social Work (LCSW). Current Licensed Clinical Social Worker licensure in the state of practice. Minimum of one year experience in a rehabilitation setting serving individuals who have experienced traumatic brain injury, spinal cord injury, or acute diagnosis. Current BLS (Basic Life Support), First Aid, and CPR certification required. Ability to lift 50 pounds. Ability to lift up to 100 pounds in moving, lifting, or transferring patients following safety procedures. Fine motor skills, visual acuity, and ability to walk/stand for extended periods. Physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently. #J-18808-Ljbffr
